Job summary

Urban Recovery is seeking a Patient Care Coordinator to support treatment engagement and ensure a safe, healing environment. You will communicate with counselors and nursing staff, de-escalate when needed, and help keep patients on the program schedule.

Responsibilities include room and census checks every hour, safe transport within the facility, admissions screening for contraband with staff, and helping with laundry and linen operations to support daily care.

Qualifications

Responsibilities

• Communicate with counselors and nursing staff regarding patient care and issues.
• De-escalate behaviorally compromised patients.
• Room and census checks every hour.
• Transport patients safely throughout the facility.
• Monitor program adherence and observe for behavior.
• Assist with admissions and screening for contraband.
• Assist with laundry and linens as needed.
