Job summary

InterCommunity, Inc. is seeking a Recovery Support Specialist to provide skill-building, engagement, and care coordination for patients in recovery across primary care settings.

You will help ensure coordinated care through our recovery system and work closely with a transdisciplinary team to review services daily. The role requires a High School Diploma, a valid driver’s license, and CPR/First-Aid certification within 6 months of hire, with prior experience in behavioral health or substance use

Qualifications

• High School Diploma is required.
• Valid driver’s license required.
• CPR and First-Aid certification is to be obtained within 6 months of hire.
• Prior experience working with people with severe and prolonged mental illnesses and/or substance abuse disorder is required.

Responsibilities

• Provide skill building, engagement, and care coordination for patients in recovery.
• Coordinate care effectively through InterCommunity’s recovery system of care.
• Participate on a transdisciplinary team with daily meetings to review client services.
• Link clients to clinical, medical, social, educational, rehabilitative and vocational services as needed.
• Support client recovery by participating in individual treatment sessions and coordinating with MAT teams.
• Create and facilitate groups based on client needs in office and community settings.
