Job summary

Southwest Network is seeking an ACT case manager to deliver assertive treatment services and coordinate individualized service plans in a community setting. You will participate in ISPs, advocate for members, and assist with discharge planning and crisis intervention.

The role requires strong communication, organizational skills, and a commitment to recovery-oriented care. After-hours availability and a valid Arizona driver license are essential.

Qualifications

• Must have one of the listed degrees or equivalents in behavioral health or related field.
• Minimum 1 year experience with seriously mentally ill adults; ACT experience preferred.
• Certifications: CPR, First Aid, Therapeutic Options; valid AZ driver license & insurance.
• Must be at least 21 years old.

Responsibilities

• Provide assertive treatment services and implement ISPs.
• Coordinate, monitor, and provide services in community settings.
• Act as member advocate and assist in crisis intervention and discharge planning.
