Job summary

Qualifications

• Associate degree preferred in related field.
• One year experience in substance use treatment or related field.
• Excellent verbal and written communication skills required.

Responsibilities

• Provides direct 1-to-1 support sessions and/or facilitates groups as scheduled.
• Engages patients with recovery-oriented activities and supports engagement.
• Assist clinical teams by providing direct patient support and continuity of care planning.
• Document all patient interactions accurately in the continuity of care record.
• Support family engagement, orientation, and family visits as needed.
• All other duties as assigned.
