Job summary

Recovery Centers of America (RCA) is seeking a proactive Case Manager to guide patients from inpatient care to community-based support. You will develop individualized aftercare plans, coordinate with clinical teams, families, and external providers, and address barriers to care to promote sustained recovery.

Your role emphasizes advocacy, thorough documentation, and collaboration with multidisciplinary staff to ensure seamless transitions and ongoing engagement in recovery journeys.

Qualifications

• High school diploma or GED; bachelor’s in social work preferred.
• Professional experience in case management and SUD treatment, with 1+ year in behavioral health.
• Knowledge of community resources and recovery supports.
• Strong problem-solving and communication skills.
• Proficiency with Microsoft Office and EMR systems.

Responsibilities

• Develop and coordinate personalized aftercare plans, ensuring patients have access to necessary resources after discharge.
• Assist patients in scheduling follow-up appointments, including primary care and SUD/mental health services.
• Advocate for patients, addressing housing, employment, legal concerns, and social support.
• Work closely with the clinical team to align aftercare plans with treatment goals.
• Engage families and support systems, providing education and resources.
• Maintain thorough documentation of discharge planning, referrals, and progress.
