Job summary

La Frontera Arizona, Inc. is seeking a counselor/case manager to provide counseling and supervision in diversas settings, including detox and residential facilities. The role supports clients through evaluation, planning, and daily living skills, within a trauma-informed framework.

The position emphasizes coordinating care with medical and behavioral health teams, maintaining records, and advocating for clients with community agencies, while ensuring safety and quality of care.

Qualifications

• Counseling individuals, families, and groups.
• Knowledge of principles and practices of case management.
• Ability to coordinate the work of others.
• Knowledge of cultural characteristics of minority groups.
• Ability to communicate effectively both orally and in writing.

Responsibilities

• Provide counseling and supervision of clients in individual and group settings as part of a multidisciplinary team.
• Maintain client records per guidelines and procedures.
• Provide supportive counseling and training in daily living skills (hygiene, nutrition, budgeting, food prep).
• Intervene in crisis situations and avert crises when possible.
• Observe and report nursing-related observations (medication effects, nutrition, infection control).
• Direct and assist clients in planning and evaluating activities.
• Advocate for clients and families with other agencies and the public.
• Transport clients in agency vehicles as needed.
• Consult and follow up with community agencies to provide needed services.
• Implement and maintain safety standards for clients and staff.
• Attend in-service trainings and staff meetings.
• May assist in evaluations, planning of behavioral treatment plans, orientation and training of staff, and assessing job skills.
