Job summary

America’s Rehab Campus (ARC) in Tucson, AZ seeks a Case Manager to provide comprehensive case management for clients in residential, detox, and outpatient programs. You will work with a multidisciplinary team to ensure continuity of care, advocacy, discharge planning, and linkage to community resources aligned with individual recovery goals.

Qualifications

• Requires high school diploma or equivalent.
• Two years of related field experience preferred.
• CPR/FA certification to be obtained within first 30 days.
• Fingerprint clearance card (or applying within 7 days of hire).
• Handle with Care certification to be obtained within first 30 days.

Responsibilities

• Develop and monitor individualized case management and service plans with clients and treatment teams.
• Coordinate medical, psychiatric, social, housing, employment, and aftercare services.
• Maintain regular contact with clients to support progress toward treatment and discharge goals.
• Assess client needs and barriers to treatment; document findings in the EHR.
• Collaborate with therapists, nurses, and physicians to ensure integrated care delivery.
• Facilitate discharge planning with referrals for continued treatment and support services.
• Ensure timely documentation of services, contacts, and case notes following standards.
