Job summary

Mohave Mental Health Clinic, Inc. seeks a Health Coach/Peer Support Specialist to guide clients in the clinic system toward greater independence in the community.

You will work as part of a clinical team, using recovery-oriented approaches and documenting services to meet RBHA and health plan standards. Applicants should have a high school diploma or GED, be at least 21, and demonstrate strong communication and teamwork.

Qualifications

• High School Diploma or GED required.
• 21 years of age or over.
• No convictions for felonies or misdemeanors involving substance abuse, sexual misbehavior or assault.
• Able to work flexible hours and arrange own transportation.
• Must obtain fingerprint clearance in Arizona.
• Must obtain CPR and first aid certification.
• Must complete Crisis to Care Intervention training within 30 days of hire.
• Must provide documentation of successful completion of Peer Support Training Course.

Responsibilities

• Provide health coaching and mentor clients within the clinic system.
• Assist clinical team in developing care plans and accessing supports.
• Model recovery principles to help clients cope with stressors.
• Teach recovery tools (e.g., WRAP, Recovery Planner) to clients.
• Help clients develop natural supports and community living skills.
• Demonstrate professional behavior and self-advocacy.
• Coordinate with supervisor on activities, supervision, and training.
• Maintain required clinical documentation per standards.
• Support clients in planning services with health coaching and peer support.
• Communicate progress with supervisor and clinical team.
• Work well under pressure and meet deadlines.
• Deliver excellent customer service to clients and stakeholders.
• Perform other duties as assigned.
