Responsibilities include collaborating with clinicians, developing care plans, and maintaining thorough documentation while supporting clients in achieving independence and stability in community

Competenze

• Requires strong verbal and written communication and collaboration with clinical teams.
• Must complete Peer Support Training and pass Recovery Competency Test; provide documentation.
• Ability to work flexible hours and maintain service levels across office and community settings.

Mansioni

• Provide peer support, health coaching, and mentoring to seriously mentally ill individuals.
• Assist in developing plans of care with clinical team members.
• Engage clients and families to build trust and ensure ongoing engagement in services.
• Demonstrate recovery tools in practice and document outcomes.
• Coordinate with supervisor and ACT team on activities, supervision, and training logs.
• Perform required clinical documentation in line with agency standards.
