Job summary

  • Requires high school diploma or GED; associates/bachelor’s in Human Services preferred.
  • Active engagement in personal recovery of 3+ years.
  • Certification in Peer Support preferred (SC course).

Responsibilities

  • Identify and screen ED patients for problematic substance use; conduct brief interventions.
  • Coordinate linkage to outpatient treatment and provide warm hand-offs to community providers.
  • Act as patient advocate to reduce stigma and support recovery.
  • Maintain post-discharge contact to address barriers to care and transitions.
  • Attend meetings and complete documentation and reports; report patient outcomes.
