Job summary

Mental Health Resource Center, Inc. is seeking a FACT-IL Case Specialist to provide treatment, rehabilitation, care coordination, and recovery support to adults with serious mental illness under clinical supervision.

As a member of a multidisciplinary team, you will promote recovery, empower self-determination, and facilitate community stability while implementing individualized recovery plans and coordinating services across settings including homes, offices, and clinics.

Qualifications

• Bachelor’s degree in Human Services or a related field is required.
• Certified Recovery Peer Specialist (CRPS) credential preferred.
• Experience working with adults with severe mental illness is required.
• Proficiency in RBHS/MHRC EHR and Patient Information System within three months of employment.
• Proficiency in Microsoft Office, Outlook and Internet use.

Responsibilities

• Provide treatment, rehabilitation, and support services in accordance with program standards.
• Complete assessments and portions of comprehensive assessments.
• Coordinate services and develop, implement, and monitor individualized recovery plans.
• Advocate for access to medical, housing, legal, financial, and community resources.
• Provide support with activities of daily living, transportation, and interpersonal skill development.
• Educate individuals about symptoms, medications, coping skills, and recovery.
• Maintain accurate, confidential clinical documentation and records.

Education

Bachelor’s degree in Human Services or a related field
