Gosnold is seeking a Case Manager/CSS for a 40-hour role based in Wareham, MA. The position starts in Falmouth and moves to Wareham this Fall. Hourly pay ranges from $24.00 to $30.00 depending on licensure and experience.

Formación

• Bachelor's degree in healthcare or related discipline is preferred.
• Minimum one year of experience in mental health, addiction, or home health/hospice settings.
• Excellent customer service, verbal and written communication skills.
• Computer literacy and familiarity with technology.

Responsabilidades

• Meet with caseload daily to assess needs and document progress.
• Conduct biopsychosocial and Columbia assessments within 48 hours of admit.
• Prepare treatment and discharge plans with patients and supports.
• Facilitate groups as assigned.
• Maintain caseload using Electronic Health Record system.
• Coordinate with referral sources and advocacy groups.
• Contribute to supervision, trainings, and certification requirements.
• Assist with reimbursement processes under supervision.
• Maintain confidentiality and ethical standards.
• Perform other duties as assigned.
