Job summary

Bay Cove Human Services in the Greater Boston area seeks a Case Manager for Residential Recovery Services to provide counseling, support, and referrals to individuals with substance use and co-occurring disorders.

You will coordinate services, develop aftercare plans, document in the Electronic Health Record, and work with a clinical team on discharge planning and relapse prevention. High school diploma required; bachelor’s degree preferred.

Qualifications

• Minimum of one year experience in substance use disorder/behavioral health treatment settings.
• High school diploma or equivalent; bachelor’s degree preferred.
• Ability to communicate clearly and persuasively.
• Knowledge of confidentiality regulations for substance use treatment.
• Proficiency with Windows, Word, and Google Apps.
• Certification eligibility for substance abuse counseling welcomed.

Responsibilities

• Orient new residents and explain program rules and rights.
• Coordinate with external providers and arrange appointments.
• Develop aftercare plans and coordinate transitions to aftercare.
• Document services and resident progress in the EHR.
• Provide crisis prevention and intervene when needed.
