Job summary

Indiana Treatment Centers in Muncie, IN is seeking a compassionate Case Manager to coordinate services, link clients to community resources, and advocate for lasting recovery within a collaborative, multidisciplinary team. You’ll manage discharge planning, track progress, and document in the EHR while building relationships with community partners and resources.

Requires a high school diploma; associate’s or bachelor’s in a behavioral health field is preferred; 1 year of related experience;

Qualifications

• High school diploma or equivalent required; associate's or bachelor's in behavioral health or human services preferred.
• Minimum of one year of experience in case management, care coordination, or direct services in behavioral health or related settings.
• Knowledge of community resources and social service systems.
• Experience documenting in an Electronic Health Record (EHR).
• Current CPR/First Aid certification or ability to obtain within 30 days of hire.

Responsibilities

• Provide comprehensive case management services for clients with moderate to high clinical and psychosocial needs.
• Coordinate discharge planning and transitions of care to ensure continuity of services.
• Facilitate interdisciplinary case reviews and communicate client progress and treatment recommendations.
• Assist clients with housing, employment, transportation, insurance, benefits, and community resources.
• Build relationships with referral sources, community agencies, probation officers, courts, employers, and other partners.
• Advocate for clients while promoting accountability and recovery.
• Maintain timely, accurate documentation within the EHR and participate in quality improvement activities.
• Support a positive, recovery-focused treatment environment.
