Job summary

Indiana Treatment Centers in Columbus, Indiana, seeks a compassionate Case Manager to coordinate services for clients with substance use disorders. You’ll connect clients with housing, employment, benefits, and community resources while advocating for long-term recovery and documenting progress in the EHR.

You’ll work within a multidisciplinary team to ensure seamless transitions of care and client success, with a focus on trauma-informed, recovery-oriented care in a supportive environment.

Qualifications

• High school diploma or equivalent required
• Minimum of one year of experience providing case management, care coordination, or direct services in behavioral health or related setting
• Knowledge of community resources and social service systems
• Experience documenting services in an Electronic Health Record (EHR)
• Strong communication, organization, and follow-through skills

Responsibilities

• Provide comprehensive case management services for clients with moderate to high clinical and psychosocial needs
• Coordinate discharge planning and transitions of care to support continuity of services
• Facilitate interdisciplinary case reviews and communicate client progress and treatment recommendations
• Assess client needs and identify barriers impacting treatment or recovery
• Connect clients with housing, employment, transportation, insurance, benefits, healthcare, and community resources
• Assist clients with obtaining identification, benefits, and other resources
• Build collaborative relationships with referral sources and community partners
• Advocate for clients while promoting accountability and recovery
• Maintain timely, accurate documentation within the EHR
• Participate in multidisciplinary meetings and quality improvement activities
