Qualifications

• ASAM Criteria knowledge required.
• Proficiency in EHR documentation.
• Strong organization and time-management skills.
• Effective verbal and written communication.
• Ability to collaborate within an interdisciplinary treatment team.
• CPR/First Aid certification within 30 days of hire.

Responsibilities

• Conduct biopsychosocial assessments and contribute to treatment plans.
• Coordinate care among clinical, medical, and recovery teams to ensure continuity.
• Provide case management for patients with moderate to high needs.
• Assist with crisis coordination and complex psychosocial needs.
• Coordinate discharge planning and prepare for next level of care.
• Engage in interdisciplinary case reviews and document progress.
