Job summary

Thrive Behavioral Health is seeking a Case Manager for their residential program to provide direct services, coordinate care, and link clients to needed services. The role involves advocacy, education, and collaboration with a broad network of care providers to support recovery.

The candidate will monitor psychiatric functioning, assist with housing and income maintenance, and develop person-centered recovery plans in compliance with Thrive standards and regulations.

Qualifications

• Bachelor's Degree in the Human Service field or an equivalent combination of education and experience required.
• Minimum 2 years experience in Human Service field working with clients with disabilities preferred.
• Certification in Case Management / Community Support preferred or working toward.

Responsibilities

• Provide assessment, referral, education, treatment, and follow-up services to clients on mental health, medical, substance abuse, vocational, and living skills.
• Collaborate with other providers, agencies, and individuals in the client's network of care.
• Provide referral, advocacy and education to the community regarding chemical dependency or mental health.
• Monitor signs and symptoms of psychiatric functioning and medication effects; document findings.
