Job summary

Indiana Treatment Centers is seeking a compassionate, organized Case Manager to join our multidisciplinary team in Williamson, West Virginia. You’ll help clients overcome barriers by coordinating services and advocating for long-term recovery.

Provide comprehensive case management for clients with moderate to high needs, coordinate discharge planning, and connect clients with housing, employment, and community resources. A supportive, trauma-informed environment is offered.

Qualifications

• High school diploma or equivalent required.
• Minimum of one year of case management or related experience in behavioral health.
• Knowledge of community resources and social service systems.
• Experience documenting services in an Electronic Health Record (EHR).

Responsibilities

• Provide comprehensive case management for clients with moderate to high needs.
• Coordinate discharge planning and transitions of care.
• Facilitate interdisciplinary case reviews and communicate progress and treatment recommendations.
• Connect clients with housing, employment, transportation, insurance, benefits, healthcare, and community resources.
• Maintain timely, accurate documentation within the Electronic Health Record (EHR).
