Job summary

Central City Concern is seeking a Case Manager to help stabilize residents within our program properties. You will assist clients in accessing addiction treatment, mental health services, housing, and self-sufficiency resources while maintaining professional boundaries and ensuring recovery-oriented support.

The role requires understanding of dual relationships and confidentiality. Schedule is Sunday-Thursday, 8:00am-5:00pm, with some travel to partner sites as needed.

Qualifications

• High School Diploma or GED with 6 months of experience in relevant settings.
• Pass a pre-employment drug screen and background check.
• Must carry an agency phone and adhere to non-discrimination policies.
• Ability to perform physical tasks and navigate stairs with or without accommodation.
• Ability to work respectfully with diverse clients and colleagues.

Responsibilities

• Support housing stability and recovery for residents within the program.
• Process resident intakes and discharges and maintain related records.
• Meet with clients bi-weekly and refer to CCC and external resources.
• Connect residents to housing, employment, and community-based services.
• Maintain privacy, complete required reports promptly, and enter data as needed.
