Behavioral Health Group - BHG is seeking a committed Case Manager to support patients on their recovery journey in Rhode Island. You will coordinate referrals, orient patients to services, and ensure continuity of care with the treatment team.

The role emphasizes compassionate, patient-centered care with a Monday–Friday 5:30 AM–2:00 PM schedule and a $21/hour base pay plus bonus potential. BHG offers strong benefits, growth opportunities, and a supportive work environment.

Qualifications

• Bachelor’s degree required or higher.
• Ability to meet federal and state certification and licensure requirements.

Responsibilities

• Provide individualized substance use disorder case management throughout recovery.
• Welcome and orient patients to treatment program services, policies, and expectations.
• Coordinate referrals and follow-up to ensure continuity of care.
• Provide crisis intervention during critical moments.
• Maintain knowledge of addiction treatment practices, regulations, and trends.
