Job summary

Behavioral Health Group - BHG in Colorado is seeking a committed Case Manager to support patient-centered care and recovery.

You will help patients with referrals, coordinate care, and collaborate with the treatment team to break the cycle of addiction and rebuild hope. This role offers a 5am–1:30pm schedule (M-F) and a pay range of $22–$26 per hour plus a bonus structure.

Qualifications

• Bachelor's degree required.
• Experience in case management or addiction recovery is a plus.

Responsibilities

• Support patients with referrals and resources.
• Coordinate care across services and providers.
• Collaborate with treatment team to plan and monitor progress.
