Job summary

The Providence Center in Providence, RI seeks a Case Manager for the BHOP Team Z2 to coordinate services and support for individuals with behavioral health needs. 37.5 hours/week, Monday–Friday, with a retention bonus opportunity and Spanish bilingual stipend.

Responsibilities include conducting assessments, developing care plans, coordinating with a multidisciplinary team, and connecting clients to housing, education and healthcare resources.

Qualifications

• Associate's degree required; Bachelor's degree preferred.
• Field of Study: Human Services or related
• Experience in Human Services field preferred.
• Must have valid driver's license, registration, current inspection sticker and insured auto.
• Bilingual Spanish preferred.

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Provide services to assigned caseload with in-person contacts in office and community settings.
• Conduct comprehensive assessments to identify housing, education, employment, healthcare, and resource needs.
• Develop, present and implement individualized care plans with care team and families.
• Connect clients to community resources and monitor progress toward goals.
• Perform crisis intervention and risk management as needed, escalating to QMHPs or Emergency Services.
