American Addiction Centers in Texas seeks a dedicated Case Manager to oversee treatment services for an assigned caseload, coordinating care from intake to discharge. You will advocate for patients, collaborate with counseling and utilization review teams, and ensure timely documentation in the electronic health record.

The role emphasizes adherence to ASAM criteria, HIPAA privacy, and professional communication with patients, families, and staff.

Qualifications

• Bachelor’s Degree in Behavioral Sciences from an accredited University preferred.
• ***State recognized (IC&RC) alcohol and drug counseling certification (or intern SUD counselor certification) minimum of CADC II or above required.***
• Minimum of 2 years experience in a behavioral healthcare field and/or substance abuse recovery field.
• Experience in behavior health treatment including group facilitation and case management.
• Knowledge of utilizing electronic health record software and documentation.
• Knowledge of ASAM criteria.
• Ability to read, write and interpret information and document clearly.
• Proficient with Word, Excel, and Outlook.
• Excellent reading, writing and interpretation of information; ability to present to groups.

Responsabilités

• To perform case management requirements in accordance with scope and laws.
• Maintain a patient caseload and coordinate care with providers.
• Provide weekly coordination to improve treatment adherence and follow-up.
• Complete aftercare assessment and discharge plans per policy timelines.
• Maintain communications with referents, family, and employers as needed.
• Attend weekly treatment team meetings and address aftercare issues.
• Ensure documentation meets policy, licensure, accreditation, and standards.
• Maintain confidentiality and HIPAA compliance.
• Communicate with other departments to ensure quality patient care.
• Attend treatment team meetings and support required supervision and training.
