Job summary

Encompass Health Corporation in Plano, TX is seeking a Case Manager to coordinate patient care from admission to post-discharge. You will work with an interdisciplinary team to shape plans, manage resources, and ensure timely, compliant care that aligns with patient goals.

This role emphasizes documenting decisions, discharge planning, and ongoing communication with patients and families, payors, and post-discharge providers, while staying current with regulations and best practices.

Qualifications

• Licensed or certified healthcare professional with independent assessment ability.
• CCM or ACM certification required or to be obtained within 2 years.
• Bachelor's degree required; graduate degree preferred where applicable.

Responsibilities

• Guide treatment plans with an interdisciplinary team based on patient needs.
• Establish tentative discharge and contingency plans with the team.
• Plan and execute the patient discharge experience.
• Monitor patient experience including quality, timeliness and payor needs.
• Lead weekly team conferences and adjust care plans as needed.
• Complete case management addendums and required docs.
• Stay current on regulations, policies and department operations.
• Review case mgmt reports and plan appropriate actions.
• Attend ACT meetings to identify trends and reduce ACTs.
• Meet patient/family within 24 hours of admission.
• Assess goals and addendums within 48 hours of admission.
• Educate patient/family about rehab and case manager role.
• Schedule and hold family conferences as needed.
• Help procure resources to avoid discharge delays.
• Monitor orthotics/prosthetics ordering and payment compliance.
• Refer to post-discharge providers and physicians as needed.
• Ensure accurate discharge and payor info in records.
• Participate in utilization review data collection and actions.
• Join case management on-call schedule as needed.
