Job summary

Avenues Recovery Center is a nationwide network of drug and alcohol rehab centers with 21 locations across seven states. We provide evidence-based clinical care and individualized support to help clients achieve recovery.

We are seeking a compassionate case manager who can advocate for clients, coordinate intake, manage insurance coverage, housing, and post-discharge planning to ensure seamless continuity of care.

Qualifications

• High School diploma or GED preferred.
• Min. 1-year experience in treatment industry preferred.
• If in recovery, a minimum one-year period of sustained sobriety is required.
• Excellent communication, multitasking, and time management skills.
• Strong problem-solving and critical thinking skills.
• Warm, empathetic, respectable personality.

Responsibilities

• Advocate for client, addressing and resolving any external barriers to treatment.
• Initiate case management for each client upon intake, addressing any pressing needs.
• Oversee client accounts, ensuring insurance is active throughout duration of treatment.
• Provide ongoing case management throughout the duration of treatment.
• Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post-discharge.
• Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.).
