Qualifications

• Typically 1–2 years of experience in substance abuse and mental health settings.
• Knowledge of DCF guidelines and Joint Commission standards.
• Familiar with 12-step recovery programs.
• Kipu knowledge preferred.
• CADC or equivalent credential preferred.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centered.
• Handle case assignments, draft service plans, review case progress and determine case closure.
• Communicate patient needs to care team members and follow up accordingly.
• Manage discharge plans upon completion of treatment.
• Work collaboratively with patients, families, therapist, and referral sources to ensure high quality care.
• Maintain the patient's comprehensive clinical record through detailed documentation.
