Job summary

Zinnia Health is seeking a full-time Case Manager, CADC/CCAPP-certified, to support clients in a substance abuse treatment setting. You will provide direct services, maintain case notes, and collaborate with clinicians to connect clients with community resources.

The role requires knowledge of 12-Step programs and EMR documentation; you will advocate for clients, manage service plans, and assist with discharge planning while contributing to a team oriented, recovery-focused environment.

Qualifications

• Bachelor's degree in human services preferred.
• 1–2 years of experience in Substance Abuse and Mental Health settings.
• Knowledge of 12 Step Recovery programs.
• Knowledge of Kipu EMR is preferred.
• Candidates holding CADC or equivalent credential are strongly preferred.

Responsibilities

• Coordinate and provide care that is patient-centered and efficient.
• Draft service plans, review progress, and determine case closure.
• Communicate patient needs to care team and follow up accordingly.
• Manage discharge plans upon completion of treatment.
• Maintain the patient’s clinical record through detailed documentation.
