Job summary

Resources for Human Development (RHD) is seeking a compassionate Case Manager with a CADC certification to join the Lehigh Valley ACT Program in Bethlehem, PA. This full-time role works within a multidisciplinary ACT team to deliver recovery-oriented, community-based services and coordinate access to housing, healthcare, and supports.

You will conduct screenings, develop recovery plans, connect participants to resources, advocate for needed services, and maintain documentation while traveling

Qualifications

• Current CADC certification required.
• Two years of experience in behavioral health, addiction recovery, homelessness services, or BHH/IDD programs.
• Intermediate computer skills and ability to complete electronic documentation.
• Strong interpersonal, communication, and customer service skills; able to work independently and in a multidisciplinary team.

Responsibilities

• Work as part of a multidisciplinary ACT team to provide holistic, recovery-focused services.
• Assist individuals in developing and achieving goals in their recovery plans.
• Connect participants to community resources, education, healthcare, housing, and recovery programs.
