Job summary

Qualifications

• Bachelor's degree in healthcare or related discipline preferred.
• Minimum one year of experience in mental health, addiction, or related field.

Responsibilities

• Meet with individual caseload daily to assess needs and progress in treatment, completing daily contact notes.
• Conduct patient biopsychosocial and Columbia assessments within 48 hours of admittance.
• Prepare treatment plans and discharge planning with patients and supports.
• Facilitate groups as assigned.
• Manage caseload and track tasks using Electronic Health Record.
• Communicate with referral sources and advocacy groups.
• Complete required documentation per Gosnold policy and standards.
