Qualifications

• Education: Bachelor’s degree in a healthcare or related discipline is preferred.
• Experience: 1+ year in mental health, addiction, home health/hospice, or related field.
• Skills: Excellent customer service and verbal and written communication; computer literacy.

Responsibilities

• Meet with individual caseload daily to assess needs and progress.
• Conduct biopsychosocial and Columbia assessments within 48 hours of admittance.
• Prepare treatment plans and discharge planning with patients and supports.
• Facilitate groups as assigned.
• Maintain documentation in Electronic Health Record and Gosnold policies.
