Job summary

Gosnold is seeking a Case Manager to conduct patient interviews, assess needs, and plan continuing care and discharge. You will collaborate with community resources and providers to support recovery and ensure adherence to policy.

The role requires knowledge of addiction and mental health recovery processes, and strong documentation in the EHR. Bachelor’s degree in healthcare and at least 1 year of relevant experience preferred. On-site position in Wareham, MA with competitive benefits.

Qualifications

• Education: Bachelor's degree in a healthcare or related discipline preferred.
• Experience: Minimum one (1) year in mental health, addiction and/or home health/hospice or related field; team-based treatment approach preferred.
• Skills: Excellent customer service and verbal and written communication; computer literacy.

Responsibilities

• Meet with individual caseload daily to assess needs and progress in treatment; complete daily contact notes.
• Conduct biopsychosocial and Columbia assessments within 48 hours of admission.
• Prepare treatment plans and discharge planning in collaboration with patients and supports.
• Facilitate groups as assigned.
• Maintain caseload and track tasks in the Electronic Health Record.
