Job summary

Qualifications

• HS Diploma or equivalent and experience in substance abuse recovery care.
• Bachelor's degree in Social Work or related field preferred; CASAC preferred.
• Familiarity with FMLA and short-term disability processes or willingness to learn.

Responsibilities

• Meet active patients for discharge planning to develop continuing care plans.
• Research aftercare options including transitional housing.
• Locate and secure housing, 12 Step meetings, providers in patients’ home areas.
• Coordinate discharge plans with families and referral sources.
• Explain payment options and financial resources to patients and families.
• Assist with FMLA and disability paperwork and guidance.
• Collaborate with quality department to send paperwork to aftercare resources.
• Maintain ongoing communication with patients and families and in team meetings.
• Facilitate referrals to ancillary services and ensure documentation is received.
• Understand health insurance benefits and payment methods used by the organization.
• Complete documentation neatly and timely.
