Wellbridge is seeking a Case Manager in Calverton, NY to support patients transitioning from inpatient treatment to ongoing care. You will develop discharge plans, coordinate outpatient resources, and assist with insurance navigation and financial support to ensure a seamless continuum of care.

The role emphasizes partnership with patients, families, and providers, including locating transitional housing, coordinating referrals, and documenting care plans.

Qualifikationen

• HS Diploma or equivalent and experience in substance abuse recovery care.
• Familiarity with FMLA and short-term disability processes.
• Familiarity with health care standards: OASAS, ASAM, JCAHO, etc.
• Demonstrated communication, critical thinking, and interpersonal skills.
• Proficiency with data management and electronic health records.
• Strong problem solving, conceptual, and analytical skills.

Aufgaben

• Meet with active patients for discharge planning and continuing care.
• Research aftercare options including transitional housing.
• Locate and secure housing, 12 Step meetings, and providers in home areas.
• Collaborate with families and referents on discharge plans.
• Explain payment options and financial support to patients and families.
• Guide patients on FMLA and disability benefits and paperwork.
• Coordinate with QA to send paperwork to aftercare resources.
• Maintain ongoing relationships with patients and families and communicate team updates.
• Facilitate referrals to ancillary services with proper documentation.
• Assist with ACA/AMA interventions with Clinical and Nursing departments.
• Understand health insurance benefits and payment methods used by the organization.
• Ensure timely, neat documentation and professional judgment.
