Job summary

Release Recovery in New York seeks a dedicated Case Manager for our Men's Program to coordinate care from admission through aftercare. This role serves as the primary liaison among clients, families, clinical teams, and referral partners, providing structure, empathy, and detailed coordination.

Qualifications

• Minimum of 2 years’ experience in behavioral health, addiction treatment, or mental health field.
• Strong communication skills, written and verbal, with sensitivity and empathy.
• Ability to manage multiple responsibilities while maintaining a high standard of care.

Responsibilities

• Provide consistent, individualized case management to clients throughout their treatment stay and transition to aftercare.
• Meet weekly with each client to assess progress and support adherence to the program’s tier system.
• Coordinate all aspects of client care, ensuring alignment with clinical treatment plans and program expectations.
• Conduct regular phone calls with family members to provide updates, support, and psychoeducation.
• Maintain open, timely communication with clinical teams, therapists, and external referents.
