Job summary

Arch Recovery Center is seeking a motivated Case Manager to join our Level 3.5 ASAM SUD residential treatment facility. You will support clients by connecting them to community resources, helping with aftercare planning, and coordinating with the multidisciplinary team to promote recovery outcomes.

Responsibilities include meeting with clients, developing aftercare plans, documenting progress, advocating for clients, and participating in treatment team meetings to support client progress and

Qualifications

• Bachelor’s degree in a human services field (e.g., social work, psychology, counseling).
• Experience in case management or addiction services preferred.
• Strong organization, communication and empathy for clients.
• Basic computer skills for documentation and communication.

Responsibilities

• Meet regularly with clients to assess needs and connect to community resources.
• Assist clients in developing individualized aftercare plans.
• Collaborate with clinicians for coordinated client care.
• Participate in treatment team meetings and provide progress insights.
• Maintain accurate notes on progress, barriers, and strengths in compliance with guidelines.
• Monitor participant engagement and address barriers with the team.
• Advocate for clients navigating community systems and resources.
• Assist with crisis intervention and referrals as needed.
• Engage in ongoing professional development and training.
• Develop treatment plans.
