Job summary

Delta-T Group, Inc. is seeking a bachelor’s level contractor to support families in the Northern Liberties area with social work and case management experience. Responsibilities include intake assessments, trauma-informed engagement, and accurate daily data documentation.

Caseloads of up to 15 families and a 9am–5pm Monday–Friday schedule are expected. Clearances (FBI, Child Abuse, state background, ACT 31) must be obtained, with on-going documentation and reporting required.

Qualifications

• Bachelor's degree required in related field.
• Experience with trauma and crisis situations preferred.
• 1 year in human services field as case manager/service coordinator/social worker.
• Ability to pass FBI, Child Abuse, State Background checks and ACT 31.

Responsibilities

• Interact with families who have accepted FES and conduct comprehensive intake assessments.
• Manage trauma/crisis situations with appropriate support and referrals.
• Complete engagement process for families referred to FES.
• Maintain timely documentation and daily data collection.

Our client is seeking a bachelor's level contractor's looking to take on assignments in the Northern Liberties area of Philadelphia with experience in social work and case management.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY

• Interact with families who have accepted FES (Family Empowerment Services) and conduct comprehensive intake assessments.


• Must have experience with dealing/coping with trauma and crisis situations


• Responsible for completing engagement process with families referred to FES.


• Responsible for timely documentation and accurate daily data collection.


• Maintain caseload of up to 15 families.


• Ability to be at assignment 9am-5pm Monday through Friday.


• Other duties as deemed appropriate by the supervisor/manager

CLIENT'S REQUIRED SKILLS AND EXPERIENCE

• Bachelor's degree in social work, psychology, human services, sociology


• Valid Driver's License


• 1 year experience working with demographic affected by traumatic experiences


• 1 year experience in human services field as case manager, service coordinator, or social worker


• Ability to complete and pass clearances: FBI, Child Abuse, State Background and ACT 31


• Must be able to produce current medical/health documents including: physical examination, Tuberculosis/TB Skin Test, CPR, and First Aid Level 2 certification, & hepatitis B screener.


• Adhere precisely to interventions in treatment plan(s)
