Job summary

The Recovery Support Coordinator at Groups in Petersburg, VA, joins a multidisciplinary care team to provide recovery support services and coordinate care across clinical, social, and community settings. Qualifications

• High school diploma required; higher degrees may be required by state.
• Minimum of two years’ experience in case management, care coordination, or related field preferred.
• Experience with substance use or mental health challenges preferred.
• Experience using Electronic Health Records (EHR).
• Valid driver's license and access to a vehicle if role includes community travel.
• Reliable internet access and strong WiFi when working remotely.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, member-centered care.
• Build trusted relationships to support members in achieving recovery goals.
• Identify goals, provide coaching, and engage members in recovery activities.
• Reengage unengaged members through intensive outreach.
• Assist new members in transitioning to Groups care model via outreach and engagement.
• Provide in-person, virtual, and telephonic support as needed.
• Maintain a shared caseload and coordinate service delivery.
• Support care pathways for high-risk or special populations.
• Link members to healthcare, behavioral health, social services, and community resources.
• Manage referrals, ROIs, and documentation to facilitate transitions.
• Engage in community outreach and advocacy.
