Job summary

Qualifications

• Associate's degree or high school with training in community services.
• Proven case management, problem-solving and advocacy experience.
• Knowledge of HIPAA and eligibility for federal/state programs.
• Valid driver’s license and basic MS Office skills.

Responsibilities

• Meet with patients on caseload within admission and track progress.
• Coordinate care across seven areas: employment, medical, housing, finances, education, recreation, and volunteering.
• Guide patients through discharge planning and step-down processes.
• Facilitate groups on living skills, growth and well-being.
• Act as liaison with community agencies and referents.
