Qualifications

• Bachelor's Degree in Psychology, Social Work, or related field.
• Two or more years of Case Management experience.

Responsibilities

• Accept referrals from therapists and assess client needs for case management, treatment, and skills training.
• Refer and link clients to community resources.
• Monitor client condition/progress and communicate with the primary therapist.
• Advocate for clients to access needed resources and preserve rights.
• Provide case management emergency services during and after hours as assigned.
• Deliver psychiatric rehabilitation services, including readiness assessment and skills training.
• Coordinate with clinicians to support treatment planning and services.
• Liaise with state and local hospitals and community partners.
• Contribute to staff education and program development.
• Track supported employment activities and support employment programming as directed by supervisor.
