Wise Path Recovery in Williamson, WV seeks a compassionate Case Manager to coordinate services and advocate for clients on their path to recovery. You will help connect individuals with housing, employment, transportation, benefits, and community resources while ensuring continuity of care.

Join a multidisciplinary team in a mission-driven behavioral health setting, focused on person-centered care, confidentiality, and trauma-informed support.

Qualifications

• Minimum of one year of case management, care coordination, or direct services in behavioral health or related setting.
• Knowledge of community resources and social service systems.
• Experience documenting services in an Electronic Health Record (EHR).
• Ability to work effectively as part of an interdisciplinary treatment team.

Responsibilities

• Provide comprehensive case management services for clients with moderate to high clinical and psychosocial needs.
• Coordinate discharge planning and transitions of care to support continuity of services.
• Facilitate interdisciplinary case reviews and communicate client progress, barriers, and treatment recommendations.
• Assess client needs and identify barriers impacting treatment, discharge, or long-term recovery.
• Connect clients with housing, employment, transportation, insurance, benefits, healthcare, community resources, and recovery supports.
• Assist clients with obtaining identification, benefits, and other resources needed for stability.
• Build collaborative relationships with referral sources, community agencies, probation officers, courts, employers, and healthcare providers.
