Job summary

Volunteers of America Northern Rockies is seeking a Case Manager to provide support to assigned clients and facilitate connections to community resources. You will assist the treatment team and participate in ongoing assessments and linkage to services.

The role requires coordination with therapists and adherence to program policies. A Bachelor’s degree in psychology or social work is required, with preference for two+ years in case management and strong computer skills.

Qualifications

• Bachelor’s Degree in Psychology, Social Work, or related field.
• Two or more years of Case Management experience (preferred).

Responsibilities

• Assess client needs for case management and link to services.
• Refer and connect clients to community resources.
• Monitor client condition and communicate progress to the primary clinician.
• Advocate for clients to ensure access to needed resources.
• Provide transportation using VOA fleet vehicles when required.
• Maintain documentation reflecting course of treatment on time.
• Participate in supervision and team meetings.
