Job summary

Qualifications

• Bachelor's degree in Psychology, Social Work or related field required.
• Two or more years of Case Management experience preferred.
• Strong computer skills and familiarity with office software.

Responsibilities

• Provide support to assigned clients and connect them to community resources.
• Assess client needs for case management, treatment, and skills training.
• Monitor client progress and communicate with the treatment team.
• Advocate to ensure access to resources and rights are respected.
• Maintain documentation and participate in supervision and meetings.
