Job summary

Gaudenzia, Inc. in Washington, DC is seeking an Addictions Counselor to support residential and recovery services. You will conduct intake interviews, coordinate care with clinical staff, and help clients plan their next steps.

Qualifications

• Minimum of an Associate Degree or full certification as an Addictions Counselor.
• Annual training requirements of 25 hours or more per year; for DC, LICSW or LPC licenses may be required.

Responsibilities

• Conduct intake interviews and orientation to care coordination prior to discharge.
• Liaise with clinical staff to review services and coordinate referrals.
• Complete Recovery Plan with client and primary clinical counselor.
• Identify health care and recovery support needs and initiate referrals.
• Assist clients in accessing next level of care and remove barriers.
• Develop and maintain relationships with services and programs (treatment, housing, employment, etc.).
• Establish and maintain care coordination services and contact schedule.
• Maintain client records and complete required documentation.
