Job summary

Gaudenzia, Inc. is seeking a care coordination professional in DC who will conduct intake and coordinate services for clients with substance use disorders. The role requires licenses such as LICSW or LPC or equivalent and an associate degree or full certification as an Addictions Counselor.

You will collaborate with clinical staff, develop Recovery Plans, and help clients access next-level care while building strong community linkages and maintaining accurate records.

Qualifications

  • DC requires an Associate Degree or full certification as an Addictions Counselor.
  • Licenses: LICSW or LPC as applicable in DC.
  • Annual 25+ hours of required training per year.

Responsibilities

  • Conduct intake interviews and orientation to care coordination before discharge.
  • Liaise with clinical staff for review and verification of services.
  • Complete Recovery Plan in coordination with client and counselor.
  • Identify health care and recovery support needs and initiate referrals.
  • Assist clients in accessing next level of care and remove barriers.
  • Develop and maintain relationships with services (treatment, housing, employment, etc.).
  • Establish and maintain care coordination schedule and contact cadence.
  • Maintain client records and complete required documentation.

Qualifications

D.C. Education/ Licensure Requirements: Education and Training: A minimum of an Associate Degree or full certification as an Addictions Counselor. Employee must have successfully completed annual training requirements of 25 hours or more per year. For DC must possess the following licenses or certifications: LICSW or LPC

Experience
  • Associates Degree: Five (5) years
  • Bachelors Degree: Three (3) years
  • Masters Degree: Two (2) years
  • Certified Addictions Counselor (CAC): Five (5) years
What You'll Do At Gaudenzia
  • Responsible for conducting intake interview and orientation to care coordination, with client prior to discharge from residential level of care.
  • Responsible for liaison with clinical staff for review and verification of recommended services, referral to clinically recommended level of care, and collateral contact information.
  • Completion of Recovery Plan in coordination with client and primary clinical counselor.
  • Responsible for identifying health care and recovery support needs with client and initiating referrals.
  • Responsible for assisting clients in getting to the next level of care (if applicable) by removing barriers, providing reminders, and conducting outreach when appropriate.
  • Responsible for developing and maintaining relationships with services and programs. These linkages will include but are not limited to treatment, health care, employment, legal, housing, child welfare, social services, peer-support providers, etc.
  • Establish and maintain care coordination services and contact schedule.
  • Maintains client records and completes all other required documentation.
