Job summary

Gaudenzia, Inc. is seeking a dedicated counselor to design, develop, implement and supervise specialized training and services for clients in our recovery programs.

The role may involve handling an independent caseload and assisting staff in counseling clients facing complex problems. The position emphasizes coordination of care, discharge planning, and linkage to community resources, with a focus on reducing barriers and improving outcomes for individuals with substance use disorders and

Qualifications

• Education: Associate degree or CAC certification.
• Experience: five years with Associate degree; three years with Bachelor's; two years with Master's; CAC five years.

Responsibilities

• Conduct intake interviews and care coordination prior to discharge.
• Liaise with clinical staff for verification of services and referrals.
• Complete Recovery Plan with client and primary clinical counselor.
• Identify health care and recovery support needs and initiate referrals.
• Assist clients in advancing to next level of care and remove barriers.
• Develop and maintain relationships with services and programs (treatment, health care, employment, housing, etc.).
• Establish and maintain care coordination services and contact schedule.
• Maintain client records and documentation; support the team as needed.
• Assist residents with program education and access to supports.
• Ensure timely responses to medical, psychiatric, and outpatient needs; coordinate care.
