Job summary

Safe Exit Initiative Inc is seeking a Care Coordinator for Jana’s Place to assist women exiting prostitution in obtaining needed services and referrals. The role emphasizes survivor-centered care, advocacy, and coordination with health and housing resources to support recovery and autonomy.

You will work with medical and clinical staff to determine referrals, document sessions, and help connect patients to MAR and psychiatric services as part of a collaborative team. Starting hourly wage is $24.

Qualifications

• Experience supporting survivors in exiting processes preferred.
• 2 years experience providing housing and resource navigation.
• Commitment to helping women impacted by CSE.

Responsibilities

• Assist patients in obtaining needed services and referrals.
• Advocate for patients within health and social systems.
• Coordinate medical/behavioral services through health insurance.
• Provide transportation and manage waitlists for the program.
• Document sessions and referrals by end of shift.
• Collaborate with Residential Counselors and Clinical Director weekly.
