Job summary

Integrity House in Newark, NJ, seeks a Care Coordinator to provide low-threshold services at the Integrity Wellness Center. We are New Jersey’s oldest nonprofit provider of compassionate care for individuals and families experiencing addiction and mental health conditions.

The role emphasizes engaging participants, conducting screenings, developing service plans, coordinating referrals, and assisting with benefits navigation in a supportive, stigma-free environment.

Qualifications

• High school diploma or GED.
• At least 1 year in care coordination/case management.
• Commitment to non-judgmental, person-centered care.
• Ability to build trust with ambivalent individuals.
• Experience in community-based or low-threshold settings.
• Experience with people using substances, HIV/AIDS, Hepatitis C, sex workers, or homelessness.
• Knowledge of trauma-informed care and Motivational Interviewing.
• Ability to assess needs, prioritize tasks, and coordinate services.
• Ability to navigate healthcare and social service systems.
• Strong documentation skills.
• Teamwork in fast-paced environments.
• Fluency in Spanish is a plus.

Responsibilities

• Engage participants respectfully and build rapport to support engagement.
• Conduct screenings and assessments across housing, healthcare, benefits, and supports.
• Develop and implement individualized service plans.
• Use motivational interviewing and risk-reduction strategies.
• Coordinate referrals to internal and external providers.
• Assist with health insurance and public benefits navigation.
• Schedule appointments and conduct warm handoffs.
• Follow up to confirm service connections and address barriers.
• Distribute prevention materials per center protocols.
• Maintain accurate documentation of interactions and outcomes.
