Job summary

Pathways Vermont is hiring a Re-entry Services Coordinator to support individuals transitioning from incarceration or under DOC supervision. Responsibilities include client-centered counseling on life skills and housing navigation, with a focus on creating pathways to permanent housing and community inclusion.

The role requires strong communication and documentation skills, a valid driver’s license, and the ability to lift up to 50 pounds.

Qualifications

  • Excellent communication, writing, and computer skills.
  • Ability to document interactions and maintain records.
  • Valid driver’s license and reliable vehicle required.
  • Ability to lift and carry 50 pounds.
  • Willingness to learn and reflect in relationships.

Responsibilities

  • Engage in relationship building with clients in a client-centered approach.
  • Provide one-to-one support and counseling on life skills, tenancy, budgeting, and housekeeping.
  • Assist clients in navigating housing paths including housing search and lease understanding.
  • Educate and advocate regarding benefits and entitlements such as Social Security and food assistance.
  • Maintain written/computer records and program documentation.
