Job summary

Trillium Health Resources is seeking a Post Care Transitions Specialist to coordinate care planning, assessments, and transitions for individuals with chronic health and behavioral health needs. This exempt role supports discharge planning from adult care homes, inpatient units, and detention centers, following evidence-based whole person care standards.

Qualifications

• Must meet NC licensure as a human services professional or RN or an advanced degree with MH/DD/SA experience.
• Must have valid NC driver’s license.
• Must reside within Trillium’s Mid State Region (Anson, Guilford, Montgomery, Randolph, Richmond) and be able to travel within catchment.

Responsibilities

• Coordinate care planning, assessments, monitoring, and referrals for TCL individuals.
• Collaborate with licensed clinicians, RNs, and peer supporters to plan discharge/transition.
• Assess needs of individuals transitioning to supportive housing to determine additional services.
• Complete assessments as needed.
• Document planning and coordination in multiple software platforms/state systems.
