Job summary

The Coalition for the Homeless of Central Florida is seeking a Rapid Re-Housing Stability Case Manager to provide client-centered case management under the SHIP RRH program. You will use a housing-first approach, document in HMIS, and collaborate with a multidisciplinary team to help clients obtain and sustain housing.

You will manage IPPs, coordinate referrals, and ensure timely case notes while maintaining confidentiality and professionalism in a dynamic urban environment.

Qualifications

• Associate degree or equivalent in social services; Bachelor’s degree preferred.
• At least 3 years of documented case management experience.
• Homeless services, housing stability, or rapid rehousing experience preferred.
• Flexible schedule including some evenings and/or weekends.
• Bi-lingual candidates encouraged.

Responsibilities

• Maintain regular contact with clients and document in HMIS.
• Develop Housing Stability Plans with clients to address barriers to housing.
• Assist with housing applications, fees, and lease negotiations with landlords.
• Assist with move-in and ensure clients have essential household items.
• Mediate landlord issues and connect clients to resources.
• Provide or arrange services to stabilize clients in permanent or rapid rehousing.
• Monitor housing stability and support crisis resolution during rental assistance.
• Maintain and submit monthly reporting data and client files.
• Schedule appointments and manage IPP progress.
